Participants Personal & Household Risk Information:


This information sheet aims to inform you of those that are most at risk should they contract COVID-19. You should identify whether you or a member of your household is at higher risk. If you are, please discuss the risk of returning to sport with your GP and the person/s delivering the activities to make an informed decision as to whether returning to the group/sport is appropriate and safe for you and your family. While research around risk is ongoing, we simply hope to inform you so that you can make the correct decision for you and your household.


If any of the following statements apply to you or your household members, then this means that you or a household members risk is increased. If it applies to a household member then you need to discuss this with your GP and the household member to make a decision on if a return to Sport is appropriate in a group setting, as it is possible to pass on the virus to a more venerable individual.

Statements that may apply to You or your Household:

  1. Had a solid organ transplant.

  2. Undergoing treatment currently or in the last 6 months for any cancer.

  3. Currently taking or in the last 6 months have taken immunosuppressant medication.

  4. Have a respiratory condition including all cystic fibrosis, severe asthma & severe chronic obstructive pulmonary (C.O.P.D.).

  5. Have any rare diseases or unborn errors of metabolism that significantly increases the risk of infections (Such as Severe Combined Immunodeficiency (S.C.I.D.), homozygous sickle cell).

  6. Pregnant.

  7. Age 70 or older.


Have one or more of the underlying health conditions listed below:

  1. Chronic (long term) mild to moderate respiratory diseases, such as asthma, chronic obstructive pulmonary disease (C.O.P.D.), emphysema or bronchitis.

  2. Chronic heart disease, such as heart failure.

  3. Chronic kidney disease.

  4. Chronic Liver disease, such as hepatitis.

  5. Chronic neurological conditions, such as Parkinson’s disease, motor neurone disease, multiple sclerosis (MS), or cerebral palsy.

  6. Diabetes.

  7. A weakened immune system as a result of conditions such as HIV & AIDS or medicines such as steroid tablets.

  8. Being seriously overweight (a Body Mass Index (BMI) of 40 or above).

  9. Are you providing caring responsibilities for anyone meeting any of the above criteria?

Should any of the above statements apply to you or your household please notify the Express Coaching Services Ltd. administration team, present on the day, & speak to your GP to allow you to make an informed decision that protects you and your household.Could this be for YOU?
